In-Office Initial Evaluation $300

 In-Office Follow-Up Evaluation $150

 Online Follow-Up Evaluation $150 

New patients must have forms completed 48 hours prior to their appointment date or their appointment will be automatically cancelled, and the patient may be subject to a $100 fee to reschedule.
